Full Job Description

We are looking for a highly motivated individual to coordinate both our Dynamic Support Register and our Care (Education) and Treatment Reviews within the Bradford district, specifically, children with a learning disability and/or autism., The post holder will hold the Care, Education and Treatment (CETR) register for Bradford District Care Foundation Trust. They will arrange and coordinate CETRs in Bradford (Children's Services), alongside the Transforming Care Coordinator.

They will coordinate Dynamic Support Register (DSR) and chair the meetings. They will provide education and training around CETRs and DSR for adult services within BDCFT

The post holder will also provide data and information to meet national reporting requirements and in support of the Transforming Care Partnership Board via a monthly CETR report
