Carer Support Worker

An exciting opportunity has presented for a motivated and enthusiastic support worker to join the Carer Support Team within older peoples mental health service. The Carer support team are an integrated team that consists of Admiral Nurses who are supported by support workers, all of which work predominately work with carers who care for people with dementia as well as families that may be experiencing carer stress due to caring for a person with a mental health condition., Our support workers work very closely with our Admiral Nurse team who will guide and allocate the workload. This will consist of making regular visits with our service users as well as liaising with other teams and organisations to ensure that the best possible care can be provided for our Carers.

The support offered may be short or long term dependent on service user needs. They work with the carer towards identifying their needs and obtaining services to help them with their caring role. This may be pre diagnosis and / or may extend beyond a person with dementia passing away.
