Full Job Description
In the role of Case Coordinator, you will:
- Play a central role in coordinating the day to day case work of the team to ensure high quality personalised casework. You will
- Work in partnership with local authorities, accommodation services, statutory and voluntary agencies to ensure that clients move on from the service into the appropriate accommodation with the correct support in place.
- Provide line management, support and supervision to Assessment and Reconnection Workers. In this non-shift role, you will work on site in Cromwell Road, West London within normal office hours Monday - Friday. However, on occasion you may be asked to work flexibly in order to support the team or a particular client on an evening or weekend. You may also be required to work across our triage hub, and other emergency hotel locations as required., Please note we're not responsible for the content of job ads, as they're posted by the recruiter. - Your experience of communicating, liaising and negotiating with internal and external stakeholders.
- Your experience of client case management and the ability to provide reconnection advice to clients experiencing homelessness.
- The ability to manage complex behaviours and motivate staff and clients to achieve outcomes with a creative and person centred approach.
- Good organisation and prioritisation skills with the resilience to achieve and meet agreed outcomes.

We are currently looking for a Case Coordinator to join our Somewhere Safe to Stay (SSTS) team in West London. The SSTS team aim is to provide a preventative service to support people in avoiding rough sleeping, or to get off the streets quickly and into suitable alternative accommodation.
