As a Case Manager, the post holder will work as part of a dynamic team in delivering an effective service to ensure safe and effective care pathways are in place for all service users from Kent a and Sussex in specialised mental health care settings, within the NHS and Independent sectors To manage the cases of service users in Adult Eating Disorders services, ensuring the values and principles of Care and treatment for all service users are integrated into the work of Specialised Commissioning and NHS England. To provide clinical support to Provider Collaborative in relation to the defined population, this will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.Main duties of the job The role supports the business in driving transformation as well as value for money in planning, commissioning and service the role is designed to build a combination of subject matter expertise and technical skills to develop · Provision of clinical expertise to the Kent and Sussex Provider Collaborative. · Ensuring effective care pathways, within and across Provider Collaborative regional boundaries · Resolution of complex or disrupted care pathways in conjunction with colleagues across England (and within UK for service users placed in services outside England) · Supporting the Provider Collaborative in ensuring that services within Provider Collaborative Regions are compliant to all specifications and guidance. · Ensuring the delivery of high-quality services that are value for money· Ensuring case management systems and record keeping are maintained and kept up to dateWorking for our organisation Abide by the standards of behaviour expected by their professional body and the standards of behaviour expected of all staff in the Trust disciplinary policy and procedure. Take responsibility for the safeguarding and protection of children and young people and vulnerable adults very seriously, as does The Trust., To deliver requirements listed above and engage and liaise with key stakeholders, in particular; · To support the delivery of day to day activities and projects · To operate in a highly political and sensitive environment · Support the portfolio of initiatives in demonstrating value for money for the Current spend through tracking, managing and delivering agreed benefits Financial and Physical Resources · Act in a way that is compliant with Standing Orders and Standing Financial Instructions in the discharge of budget management responsibilities. · Responsible for making recommendations, providing advice and able to prepare strategic reports/briefings for the Head of Department, Steering/Reference Groups and others as required. · Ensuring that the spend on specialised mental health is clinically effectively used and to ensure the minimisation of delay and associated costs Staff Management · Responsible for undertaking appraisal and personal development including progressing any disciplinary or capability issues. · Forge close positive working relationships, in order to support an effective matrix approach to achieve NHS objectives. · To support, motivate and develop staff within the team to ensure that they are able to deliver the new responsibilities of the NHS strategy. · Managing third parties to ensure deliverables are met in a timely manner and within budget. Information Management · Drafting reports summarising status on issues, appraising outcomes, and providing progress reports for the Head of Department. · Collate as required, qualitative and quantitative information and lead appropriate analysis to develop robust business cases and contribute to Project 'products' · Analyse, interpret and present data to highlight issues, risks and support decision making. Policy and Service Development · Responsible for proposing and drafting changes, implementation and interpretation to policies, guidelines · Proposes changes to own function making recommendations for other service delivery. · The post holder will need to maintain a good knowledge of emerging policies. This will assist in the thinking and implementation of Policy and strategy. Research and Development · Plan, develop and evaluate methods and processes for gathering, analysing, interpreting and presenting data and information · Deliver projects to comply with key performance indicators. · Co-ordinating Research & Development initiatives, delegating as appropriate. Planning and Organisation · Contribute to the development of performance and governance strategies and the development and implementation of improvement programmes · Contribute to short, medium- and long-term business plans, achieving quality outcome

  • Registered Mental Health/Learning Disability Nurse or other registered health or social care professional
  • Possess a relevant additional professional qualification e.g., masters level, or equivalent level of experience of working at a senior level in specialist area., Detailed knowledge of legal issues pertaining to the specialist service including MHA 1983, S117 after care, Transforming care, Safeguarding
  • Detailed operational knowledge of specialised services, including risk assessment/management
  • Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to masters level equivalent.
  • Evidence of post qualifying and continuing professional development
  • Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this engagement, You must have appropriate UK professional registration.
