Case Manager

We are working closely alongside a Local Authority in Islington to assist with the appointment of a Case Manager, on a 3-month contract, likely to be extended at clients discretion. Please apply with your CV for immediate consideration.,

• Ensure that any commissioned packages are cost effective, innovative, using strength-based practice values and focussed on recovery
• To carry out duties and responsibilities in accordance with the Councils Health and Safety Policy and relevant Health and Safety legislation
• Deliver strengths based quality conversations, signposting and early intervention and prevention which enable our residents to lead healthy independent lives outside of formal adult social care wherever possible
• To undertake duties as directly by Service Lead or any other senior as directed and as service demands.
• Following requisite training, to assess for specialist equipment and minor adaptations as required and order same through appropriate means.
• To be committed to the Councils CARE values and ASC principles to demonstrate this commitment in the way duties are carried out.
