Full Job Description

This post is based in the East Kent area. As a Case Officer within the Short-Term Pathways Team, you will be working with people over the age of 18 with a wide range of needs. You will manage a small caseload requiring assessment, review, and arrangement of on-going support to make a real difference in achieving improved outcomes and promoting independence. Focusing on planning ongoing care and support for adults who have recently had a hospital admission, this role is within a team that works collaboratively with health colleagues and other partnership agencies and focuses on promoting a person's independence. Whilst mainly working from home at the current time, a fixed working base will be discussed at interview. The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential. The new Adult Social Care and Health service operates with modernised office processes and performance management systems. It is increasingly accessible to the public outside of traditional office hours, as we move towards an 8am to 8pm, 7 days a week service to ensure service needs are met. This post is considered by KCC to be a customer-facing position. The Council therefore has a statutory duty under Part 7 of the Immigration Act (2016) to ensure that post holders have a command of spoken English/Welsh sufficient for the effective performance of the job requirements.
