The Role:
As a Case Worker in one of our Learning Disabilities Community teams, you'll use a strengths-based approach to assess an individual's needs and provide them with support and guidance to maximise their wellbeing. Strengths-based practice focuses on people's resourcefulness, strengths and abilities to enable them to develop the best strategies to face life's challenges and live independently and safely in the community. Supported by experienced colleagues and qualified social/health care professionals, you'll use your effective relationship management and negotiation skills to collaborate with individuals, their families, providers and other health care professionals.

Travel Requirements:
