CDC Pathway Navigator

We have an excellent opportunity for an enthusiastic and motivated individual to join our Community Diagnostic Centre as a Pathway Navigator to support our Service team to implement early diagnostic pathways for patients.
This unique role is specially designed to support Best Timed Pathways and will require the post holder to support pathways as they continues to evolve over time.
You will be involved from the beginning of the pathway working both directly and indirectly with patients, assisting the clinical team in coordinating the patient pathway.
You will have excellent listening skills and be able to deal with people both internally and externally in a sensitive and professional manner and be confident using patient administration systems and database packages., As a large NHS trust we are committed to enhancing the health of our local population by delivering consistently high standards of care and working closely with local authorities and key partners. You will have experience of working in a customer focused environment, and have exceptional organisational skills and have the ability to multi-task and remain calm under pressure.
You will have a good eye for detail and accuracy and want to make improvements for our patients and you should have knowledge of providing excellent customer care.
The ability to maintain confidentiality and adhere to Trust policies is paramount.
Excellent team working, organisational and communication skills are all essential for this post
The role is demanding so you must be confident, assertive.
