Full Job Description

Are you an experienced RGN with in-depth knowledge of Continuing Healthcare (CHC) processes? We're looking for skilled CHC Assessors to conduct comprehensive reviews, ensuring that patients with CHC funding continue to meet eligibility requirements. Your expertise will be essential in helping to clear a backlog of CHC reviews across multiple nursing homes, directly improving the quality of care for some of the most vulnerable patients. Key Requirements:

  • RGN qualification with significant CHC assessment experience
  • Deep understanding of CHC eligibility criteria and review processes
  • Strong communication and organisational skills
  • Proactive approach to managing and reducing review backlogs
  • What You'll Do:
  • Conduct thorough CHC reviews for patients with existing funding, verifying their continued eligibility and ensuring compliance with CHC standards.
  • Manage a backlog of CHC cases, working efficiently and accurately to streamline processes and maintain continuity of care.
  • Engage with patients, families, and healthcare providers, communicating CHC criteria and review findings with empathy and professionalism.
  • Provide clear, well-documented assessments to support ongoing patient support or transition as needed.
