CHC Clinical Assessor

On 1 July 2022, NHS North Central London Integrated Care Board (NCL ICB) was established. The ICB is part of North Central London Integrated Care System (ICS). We welcome and encourage applications from all candidates regardless of age, race, sex or gender identity, religious beliefs, marital status, sexual orientation, disability, caring status or neurodiversity., + Undertaking robust DST assessments. + Completing timely CHC and FT reviews. + Undertaking duty work, providing specialist nursing advice and support to patients with complex care needs, their families, or other providers, clinicians or professionals involved in the care, using evidence-based practice. + Plan, implement and evaluate care plans for patients and their carers using evidence based and person-centred principles. + Case Management and case management reviews. + Working both autonomously and collaboratively. Main responsibilities + Liasing with families, providers and local authority partners using a high level of interpersonal and communication skills. + Undertaking End of Life care reviews, following national guidance for individuals Fast Tracked through the CHC system. + Exercising professional judgement as necessary when communicating information on behalf of the CHC service. + Providing specialist clinical advice to secondary care, primary care, nursing homes, domiciliary care providers to facilitate the CHC process. + Interpreting national and regional guidance relating to the implementation of the National Framework for Continuing Healthcare.
