Full Job Description

NWL Integrated Care Board is looking to appoint two full time Complex Case Managers. The post holder will report to the CHC Complex Clinical Lead. The post holder is required to have a sound track record of delivering daily operational services ideally from an acute and or community background, additional experience of care homes and domiciliary care would be beneficial to the role.

Be responsible for leading on the key operational day-to-day areas of work relating to CHC for people. Ensuring that the CHC service is fully compliant with and following the national frameworks.

Holding a CHC complex case load for adults who are CHC eligible

Support joint working with the LA ICBs and clinical teams within NWL

As an experienced nurse or AHP you will have a working knowledge of Mental Capacity Act, Safeguarding, experience of undertaking assessments, case and risk management and personalised care agenda will be essential for this role

You will be required to complete Community Deprivation of Liberty applications and attend the Court of Protection on behalf of the organisation as necessary with Senior Management and Legal support.

We are looking for innovative, forward thinking, enthusiastic and confident nurses or AHPs to be part of the skilled and dynamic team. Previous experience of working in NHS Continuing Healthcare would be an advantage and a level of knowledge and experience of the framework is required.

The post-holder will undertake various functions relating to the National Framework for NHS Continuing Healthcare which will include undertaking health needs assessments, facilitation of multi-disciplinary assessments, reviews, supporting individuals with case management and staff leadership and management.

You will be an excellent communicator with sound interpersonal skills. You will need to have the ability to manage emotive and sensitive issues with tact and diplomacy, work well under pressure, negotiate with confidence, and maintain a high level of productivity. You will become part of a diverse multi-skilled team, who support each other with their own clinical knowledge to ensure the best personalised care for each individual client.

You will complete all aspects of NHS Continuing Healthcare and strive to improve processes and services. You will respond and report safeguarding concerns working closely with system partners. You will ensure that DoLS compliance is followed. You will be required to demonstrate time management skills in your ability to manage a caseload, and work in a flexible manner within a pressurized environment.

NHS Continuing Healthcare is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS. NWL Integrated Commissioning Board are looking for individuals to join the service in a variety of roles and bands.
