CHC Nurse Coordinator

Successful applicants will need to be able to work independently to deadlines, have excellent communication skills to engage patients and/or their representatives and be able to produce large, detailed written reports. You will therefore, need to be proficient in the use of Microsoft Word. There is also a continued need to complete remote assessments via Microsoft Teams, confidence with IT systems and leading meetings both face to face and remote is essential. You will need to be a car driver as assessments can take place in clients homes, community hospitals and care homes across Herefordshire or Worcestershire and out of area where required. There is an opportunity to work flexibly to meet the needs of the service and this includes some working from home. This a Monday Friday service, across Worcestershire and Herefordshire, based in either Worcester or Hereford. We have offices in Worcester & Herefordshire and promote hybrid working across the office and working from home.
