An exciting opportunity has arisen within our Continuing Health Care Teams across Greater Manchester. We are looking for highly motivated and experienced RMNs, RGNs OR Allied Health Professionals who can work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC). You will undertake comprehensive person-centred assessments so CHC and FNC eligibility can be determined.
Roles involve assessing patients for CHC, holding a caseload, and reviewing patients, as necessary.

There are six locality place bases with current vacancies: Manchester, Bolton, Oldham, Stockport, Tameside & Trafford. NHS GM offer fully flexible roles open to all Allied Health Professionals and Registered Nurses.
The post holders will play a key role in supporting the All-Age Continuing Health Care Service across Greater Manchester.
The post holders will assist and support in the facilitation of multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures, ensuring the National Framework for Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes.
Our hours of work are Monday - Friday, working flexibly but ensuring core hours are met within the locality. There is an opportunity for Hybrid working which is a combination of face to face, office and home working., NHS Greater Manchester is a fully authorised Integrated Care Board (ICB) responsible for the full range of ICB statutory duties and powers. We're bringing together all the different organisations that support people's health and social care, so that services can join up and act faster when people need support.
Integrated care means starting with the person, understanding they will have different needs as they move through life, and connecting them with the right care.
This new NHS organisation, overseen by a Board, is in charge of the NHS funds and making sure services are in place to put plans into action. Made up of representatives from the NHS and the local council. The partnership operates at three levels: neighbourhood, locality and Greater Manchester and has a single vision and strategy. Hospitals, GPs, community services, voluntary services and others have come together to form 'provider collaboratives' within all 3 levels, helping to join care and help people live well across our ten boroughs., · The post holder will be responsible for undertaking Specialist Health Needs assessments for individuals who are undergoing assessment for, or who are already eligible for NHS Continuing Healthcare and NHS Funded Nursing Care.
· The post holder will be required to work with the individual and/or their representative, and to liaise with private providers, partner organisations, along with other health and social care professionals to ensure that all assessments are person centred.
· The post holder will be required to work flexibly to support the Individualised Commissioning team by participating in the 'Duty' rota, supporting with any urgent referrals or any safeguarding investigations.
