CHC Senior Practitioner - Childrens Nurse Coordinating the process of determination of eligibility for NHS funded Continuing Care within the parameters of the National Framework for Children and Young Peoples Continuing Care (2016) Deputise for Lead Nurse in issues around Continuing Healthcare, NHS Funded Care and Complex care. Represent NHS GM Integrated Care at multi agency meetings. Liaise with other professionals, providers and key stakeholders where required. Liaise with providers over concerns raised at review and instigate safeguarding procedures where necessary. Liaise with patients families, and carers with regard to National Framework for Children and Young Peoples Continuing Care and review process. Manage a caseload of patients who have been assessed as eligible for NHS Continuing Care or a jointly funded package of individually commissioned support.
