Full Job Description

The post-holder will
· Be responsible for the management of a defined caseload (patients/service-users) and will provide highly specialist nursing support, advice, education and/or care to children with complex needs and physical or neuro-disability. This will be both in and external to nurse and consultant led clinics.
· Carry continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of programmes of care CDS patients whilst supporting families where appropriate.
· work towards the SEND agenda on behalf of the trust and promote collaborative working between the trust, education establishments and social care services. Paying special attention to the SEND Code of Practice (2020) the job holder will work to provide a key worker for 14-21-year-old that meet referral criteria.
An important element of the role will be to ensure that the multiple services/agencies often involved in the care of the CYP are joined up and communicating effectively with the CYP's voice being at the heart of decision making. Maximizing independence opportunities, involvement, and choice for the CYP on a PFA pathway will also be an important aspect of this role.

Are you a Registered Nurse (Child) or Registered (Learning Disability) who is passionate about providing an excellent experience to Children and Young People (CYP) with Special Educational Needs/Disability (SEND) and physical disability?
We are looking for a friendly, dynamic and proactive nurse to join our team in a newly formed dual role, advocating for and supporting CYP with SEND and physical disabilities and their families, to navigate services during their transition from children to adult services with a focus on a seamless transition. The successful applicant will work in conjunction with the established child development team and will manage the preparation for adulthood practitioners.
The successful applicant will possess excellent communication skills and be able to support young people from the age of 14 to 21, maximizing their opportunities for independence and choice.
A good understanding of section 8 of the SEND Code of Practice would be beneficial to applicants.
