Build and maintain relationships with patients and families with consideration to health inequalities and diversity Participate in outreach activities to raise childhood vaccination awareness and uptake Work closely with other health, social and educational care professionals, such as local schools, childcare providers, to identify and support patients making sure that access to appropriate vaccinations is available to them and ensuring that their needs are met
