Full Job Description

North West London Childrens Continuing Care team is looking for a forward-thinking individual to join our Children's Continuing Care Team, a registered professional who is experienced in working with children & young people who have complex physical & mental health needs.

You will join a friendly, dynamic, and experienced team to help meet the statutory responsibilities under the Children's Continuing Care National frameworks, working collaboratively with partners in Education and Social care to meet the holistic needs of children and young people.

You will assess, case manage, and support the healthcare needs of children and young people with complex health needs. A high level of assessment skills is required to establish eligibility so that we can provide an appropriate level of support to the children, young people, and their families. Good communication skills are required for this role and the ability to work as part of a team. You will manage a caseload of patients effectively to quality assure Children's Continuing Care needs of children and young people on your caseload.

Main duties of the job

In your role, alongside excellent assessment and co-ordination skills, we are looking for individuals with a strong commitment to partnership working to ensure our clients receive full care provision according to individual needs. The post holder will ensure the provision of seamless, integrated services or packages of care for individuals who meet children's continuing care criteria. Each package varies according to the health need support required and, we are looking for a professional who can think with, and support families to achieve their positive health outcomes.

Successful candidates will be joining a fast-paced team that employs the use of holistic nursing, inquisitive thinking & a focus on high quality. The nature of the role is autonomous & challenging but with the benefit of supportive colleagues.

This is a role for someone who is invested in supporting children & young people to improve their health outcomes & quality of life. We are an innovative team that likes to use new ideas to continually improve practice, working in partnership with other agencies to achieve this.
