Full Job Description

Join our dedicated team as a Paediatric Hospital at Home Nurse, where you'll play a vital role in the Community Children's Nursing Service and join the team as we expand into Camden and South Barnet. As a member of the Hospital at Home team, you'll provide expert nursing care for sick children within the familiar surroundings of their own homes. Your responsibilities include autonomous working, comprehensive assessment, precise planning, implementation, and evaluation of personalized nursing care for children under the guidance of the Band 7 Team Lead, Children's Community Matron and lead consultant.
Who are we?
We are a team of specialist nurses that provide assessment, support, and advice for unwell infants, children, and young people recovering at home.
What we do:
The hospital at home nursing team support children's recovery at home, preventing admission to hospital or allowing for early discharge from hospital. We provide specialist paediatric nursing care in the comfort of the child's home.
Who can we look after?
0-18-year-olds living within Islington, Camden and South Barnet, referred to our service by hospital clinicians for management of an acute illness. Some conditions we treat are resolving infections of IVABs, jaundice, bronchiolitis, viral induced wheeze, diarrhoea and vomiting and infant feeding support., The post holder will have a leadership role and take responsibility for case-finding, team co-ordination and organisation of referrals.
We welcome applicants from a range of backgrounds including emergency nurses, ambulatory care and acute wards. It is an exciting time to join our team and be supported to enhance your scope of autonomous practice.
The service runs from 8am - 8pm, 7 days a week, throughout the year. As an integrated care organisation, there are varied opportunities for training and development plus from other specialists. The induction period provided is tailored to the post holder's needs.

+ Ensure you are familiar and comply with local protocols and systems for information sharing.
+ Know the appropriate contact numbers and required reporting lines.
+ Participate in required training and supervision.
+ Comply with required professional boundaries and codes of conduct
