Full Job Description

An exciting opportunity has arisen to join the Children's Continuing Care Team covering North Yorkshire and York place as part of Humber and North Yorkshire Integrated Care Board. As a Childrens Continuing Care Nurse you will work as part of the team but also as part of the wider multi-disciplinary teams in order to complete high quality holistic assessments, reviews and case management service across North Yorkshire and York for children and young people who may be eligible for Childrens Continuing Care.

We are looking for an organised, compassionate and motivated registered childrens nurse to join our friendly team. Experience of working with children, young people and families, good communication and IT skills are essential to this role.

You will work closely with families to support the delivery of individualised care in the community for children and young people with continuing care needs. This role requires a compassionate approach in order to act as coordinator of a multidisciplinary team to establish eligibility using the NHS National framework for children and young people. Experience of working with children and young people with learning disabilities, autism, complex and long-term health conditions is required.
