Full Job Description

The CCC team part of the All-Age Continuing Care service for the North East and North Cumbria Integrated Care Board covering fourteen Local Authorities. The CCC service undertake assessments as outlined in the National Frameworks for Children's Continuing Care utilising a partnership approach. The children and young people's health assessor role have a key role in leading the multi-disciplinary assessments, case managing and commissioning packages of care for children and young people eligible for CCC funding. The health assessor work in partnership with children, young people and their representatives, other professionals including health, education, social care and the voluntary sector. The health assessor will also be supporting the expanding all age continuing care team and ensuring the key performance indicators for the ICB are achieved. The role is the first point of contact for children/young people whilst maintaining patient confidentially at all times and each nurse has a case, This role will be responsible for:

Clinical assessments of the health needs of children and young people to determine eligibility for CCC funding.

Coordinating and completing MDT assessments and reviews in accordance with the National Framework.

Provide a high standard of holistic health assessments within the agreed timeframes.

Commissioning of the packages of care to meet the unmet health needs of children and young people eligible for CCC.

Supporting children and young people with the offer of a personal health budget to meet the health outcomes.

Case management for children and young people eligible for funding via CCC.

Record keeping as per NECS policy and procedures.

Working in partnership with children, young people, and their representatives

Building and maintain relationships with Local Authorities, providers, Foundation Trusts, and the voluntary sector

Contributing to the ICB's achievement of their statutory duties

Supporting the transition of young people to adulthood

Excellent communication skills

Excellent organisational skills

Experience of undertaking health assessments and identifying health needs

The key attributes for a team member

Experience of systems including Microsoft programmes, clinical systems

Motivation to improve the outcomes for children and young people

Ability to work as part of a team and with minimal direction

Ability to organise your own workload and prioritise
