Full Job Description

We are looking for a highly motivated and enthusiastic nurse with excellent communication skills and good organisational skills and leadership skills, Who can operate as an autonomous practitioner, delivering evidence, based nursing care to children and their families. You will support the team leader in managing team members along with innovations to continue to develop the team.

The Children's Palliative care team supports children in the community with life limiting and life threatening conditions. Although this is a seven day service you would be expected to operate over four days, from 9am-5pm. You will be part of a 24 hour on call rota, when it is necessary for a child to receive end of life care at home.

You will be part of a forward thinking and friendly team, providing holistic care to families within the community. You will be expected to work closely with our short break facilitators and other professionals within the hospital, community and wider team.

The individual would support families and patients in there own home and in the hospital setting. Acting as patient advocate and coordinating care. Clinical skills are important, along with active listening skills and good communication.
