Full Job Description
To deliver a comprehensive medicines optimisation service to patients and the multidisciplinary treatment team responsible for their care, in line with CNWL Trust policies, procedures and standards
To develop the clinical knowledge and skills to practice competently as a foundation pharmacist under the Diploma in General Pharmacy Practice
To deliver a safe, and effective dispensing and supply service to St Charles Hospital when required, as per rota.
To deputise for other pharmacists in their absence.
Delivery of medicines reconciliation and discharge counselling.
To provide input into policy and procedures affecting the use of medicines at local and Trust wide level under direction of the Advanced Specialist Pharmacist.
To act as a Practice Supervisor in the training and education of trainee pharmacists and junior pharmacist
To participate in the pharmacy on-call service (Emergency Duty Cover, bank holiday rotas) and a Saturday service at the St Charles Hospital site.
We have an exciting part-time opportunity for a motivated and enthusiastic clinical pharmacist working in our community intermediate care unit, which provides intermediate care for rehabilitation and step-up beds for patients requiring short-term inpatient care.
In this stimulating role you will build on your previous experience, developing greater autonomy and responsibility whilst providing a high-quality clinical pharmacy and medicines optimisation service to patients., Excellent clinical knowledge, attention to detail and good time management are essential. You must be a pharmacist registered with the General Pharmaceutical Council (GPhC) and have completed or be working towards completion of a Postgraduate Diploma in General Clinical Pharmacy.
You will need to demonstrate good written and verbal communication skills, have good organisational skills and an ability to work independently and as part of a multidisciplinary team.,
- Vocational Master's Degree in Pharmacy
- Registered with the General Pharmaceutical Council
- A post graduate certificate in general Clinical Pharmacy
- Working towards higher Degree / Diploma in Clinical Pharmacy
- Member of the RPSGB
- Working towards membership of the faculty of the RPSGB
- Demonstrate the ability to appropriately recommend, substantiate and communicate medicine related information to patients, carers and clinical staff
- Recent significant and relevant experience in a Hospital Pharmacy.
- Previous evaluated experience of mentorship and training skills
- To have undertaken clinical audit / quality improvement
- Training and education
- Experience in clinical pharmacy for community health services and / older people / medicine and neurology / stroke
- Staff management experience
- Monitoring and advising on drug expenditure
- Clinical and critical appraisal skills
- Teaching and presentation skills
- Communicates with medical, nursing, and other MDT & pharmacy staff in clear precise and appropriate manner.
- Demonstrates awareness of the clinical governance agenda
- Evidence of writing procedures, guidelines or protocols.
- Able to influence medical staff & nursing staff
- Demonstrates ability to meet set targets
- Demonstrate ability to organise self in order to ensure efficient use of time.
- The ability and willingness to deputise for senior staff while recognising limitations of experience and ability
