Full Job Description

To deliver a comprehensive medicines optimisation service to patients and the multidisciplinary treatment team responsible for their care, in line with CNWL Trust policies, procedures and standards

To develop the clinical knowledge and skills to practice competently as a foundation pharmacist under the Diploma in General PharmacyPractice

To deliver a safe, and effective dispensing and supply service to St Charles Hospital when required, as perrota.

· To deputise for other pharmacists in theirabsence.

· Delivery of medicines reconciliation and dischargecounselling.

To provide input into policy and procedures affecting the use of medicines at local and Trust wide level under direction of the Advanced SpecialistPharmacist.

To act as a Practice Supervisor in the training and education of trainee pharmacists and juniorpharmacist

To participate in the pharmacy on-call service (Emergency Duty Cover, bank holiday rotas) and a Saturday service at the St Charles Hospitalsite.

We have an exciting part-time opportunity for a motivated and enthusiastic clinical pharmacist working in our community intermediate care unit, which provides intermediate care for rehabilitation and step-up beds for patients requiring short-term inpatient care.In this stimulating role you will build on your previous experience, developing greater autonomy and responsibility whilst providing ahigh-qualityclinical pharmacy and medicinesoptimisation service to patients. The successful candidate will be working with the wider multidisciplinary team of nurses, doctors and therapists., Excellent clinical knowledge, attention to detail and good time management are essential. You must be a pharmacist registered with the General Pharmaceutical Council (GPhC) andhave completed or be working towards completion of a Postgraduate Diploma in General Clinical Pharmacy. You will need to demonstrate good written and verbal communication skills, have good organisational skills and an ability to work independently and as part of a multidisciplinary team.

Applicants who are registered pharmacists, with experience in hospital pharmacy, and a strong interest in care of the elderly/ intermediate care are encouraged to apply for this post.
