Full Job Description

Job Overview: The main responsibilities will be to process all incoming correspondence (electronic and paper copies) to summarise and read code relevant information

Review all incoming documents and forward to clinical team as appropriate.

Filing documents and entering pertinent information into the patients' electronic medical record in accordance with the Coding Protocol.

Utilize coding systems and software applications (e.g., EMIS WEB , SNOMED Clinical Codes) to accurately assign codes and ensure the integrity of coded data.

To summarise patient medical records in accordance with the Practice's protocol

Duties:- The following are the core responsibilities of the clinical coder. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels.

a. Accurately clinically coding data on EMIS into the patient record

b. Identifying relevant clinical information documented in patient correspondence, coding appropriately and completing any detailed action

c. Inputting and extracting data as required to support practice reports

f. Actioning all incoming correspondence, emails and mail in a timely manner

g. Scanning patient related documentation and attaching scanned documents to patients' healthcare records.

h. Monitoring progress of notes summarising against practice targets.

i. To understand clinical codes and how to search for codes on EMIS clinical system. To have a basic understanding of medical terminology.

j. To share knowledge of speciality terminology with other staff where needed.

k. During absence of colleagues provide essential cover and carry out any other appropriate duties to ensure continuing provision of the service.
