Fixed Term contract/secondment opportunity for up to End of March 2025

  • An exciting opportunity has arisen to join the GHC Leadership Team for CMHT Transformation. Working with colleagues with lived experience, commissioners, primary care networks and VCSE organisations. The post will focus on GHC community team engagement and supporting existing teams to moved towards a more integrated team approach with VCSE and primary care as locality networks across Gloucestershire. This is a fast-moving program of change with opportunity to make a real difference in how future services are designed and led. Coproduction and service user/carer/professional design are key principles of the programme., The Clinical Implementation Manager for Community Mental Health Transformation (CMHT) Programme. The post holder will clinically lead the delivery of the transformation programme across GHC community teams, primary care and VCSE providers. The delivery objectives are:
  • Align community mental health teams to the primary care network
  • Implement a clear entry route into core community mental health services
  • Support the creation, review and implementation of a Trusted Assessment Process in core community mental health teams.
  • Build a neighbourhood approach to Community Mental Health support with our Voluntary Community Social Enterprise (VCSE) partners.
  • Develop the CMHT Model of Care to clearly communicate with the public and our partners what we deliver within the CMHT.

    We are looking for an experienced mental health professional with enthusiasm and a proven record of engagement and organisational change.
