The North East Hants and Farnham CMHT for older people has an exciting new opportunity for a Clinical Lead nurse! This is a new post to the service and we will be developing the role over time as the successful applicant embeds into the Service. This is an ideal role for a nurse who is experienced and passionate about providing care for older adults who are living in care homes.
This involves being part of a multi- disciplinary team providing the clinical leadership and clinical supervision of clinicians within the Care Home Pathway. There will also be an expectation that the successful applicant will also carry a very small case of higher risk/complex cases. The role is ideal for a nurse who is looking to further progress into a clinical leadership role and develop their skillset in older adults care.
The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role., + The post holder will lead on the setting, monitoring and maintaining of standards of care within the Care Home Pathway Team and will advise the Sector Manager on clinical issues which may have a negative effect on the standards of clinical practice being delivered.
+ The post holder will work closely with the Sector Manager and Advanced Practitioner and provide cover in their absence. They may at times have to take on some of the operational management responsibilities should the need arise.
+ Provide supervision to staff and clinical oversight for staff in the Care Home Pathway.
+ Responding to queries from within the organisation and external agencies such as care homes and social care services.
+ Manage a small caseload of complex cases within the CMHTOP
