The Acute Care at Home Service, Staffordshire, is looking to recruit a highly motivated, enthusiastic Advanced Care Practitioners (ACP) with extensive experience to undertake the role of Service Clinical Lead, working alongside the current service clinical lead. The successful candidate will be innovative and dynamic in their leadership approach.

The successful candidate will provide strategic clinical leadership, supporting the Acute Care at Home Consultants, Head of Operations and Clinical Operational Managers to develop and maintain a gold standard acute care at home service. They will provide specialised, expert advice, education, and clinical expertise in order to meet the service vision and objectives.

Main duties of the job

The post holder will provide effective professional leadership and management for the Acute care at home Service Clinical teams which include the Unscheduled Care Call Centre (UCCC), Community Rapid Intervention Service (CRIS), Virtual Ward, Pharmacist and Quality Lead. The post holder will be a key stakeholder regarding policy, governance, service innovation and evaluation using advanced skills such as critical analysis and synthesis of information, advanced decision making, and change management.

The post holder will be the Service lead for advanced clinical practice and will be responsible for the Service development, recruitment, training, supervision and retention of the Advanced Clinical Practitioners (ACP) within the Community Rapid Intervention Service (CRIS). The post holder will govern the standardisation of Advanced Practice/Advanced roles within the Acute Care at Home Service determining a training programme for Advanced Practice.They will ensure service compliance with Health Education England Multi-Professional Framework (2017) to ensure a robust, supported workforce from trainee ACP to qualified and beyond focusing on the four pillars of advanced clinical practice, as supported by key stakeholders across the collaborative partnership.
