Full Job Description

  • To provide direction within the caseload in consideration of the impactof current health and social care agendas and local initiatives

  • To focus on the delivery of person centred care

  • To provide effective, evidence-based care to a defined populationthrough case load management and monitoring of care standards

  • Provide leadership including clinical supervision to team members and provide support to the Out of HoursCommunity Team managerial lead.

    CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care - including the provision of three GP surgeries.

    You will be somebody who is adaptable & flexible to meet the needs of the service, looking to develop in their career, with or without Community Nursing experience that has transferable skills.

    You will be somebody who is willing to develop in advanced leadership skills along side their clinical skills.
