Full Job Description

We are pleased to announce an opportunity for either a 1 year fixed term contract or " Acting-up" / Secondment as a Band 7 therapy team lead to join our Interim Care Assessment Team working across Christchurch and Bournemouth hospitals. We are looking for a dynamic and autonomous therapist to jointly lead the interim care assessment team on a job share basis.

The Interim Care Assessment Team is a multidisciplinary community team consisting of therapists, social workers and support staff; working from the acute hospital providing a discharge to assess/early supported discharge service. This is an opportunity to gain valuable experience of a "end to end" pathway for patients leaving hospital,

This post is an opportunity to personally/ professionally develop in line with the national agenda for quality hospital discharge and integrated working with key community and local authority partners, enhance your leadership and team management skills and experience. The hospital discharge landscape is changing and there will be an opportunity to get involved in transformational work. This role will also involve community assessment and treatment of patients recently discharged from hospital focusing on promotion of independence and reducing reliance on domiciliary care.
