Full Job Description

  • Provide basic and complex warfarin dosage management in the initiation and management of warfarin therapy
  • Lead and manage clinics for complex and medically unstable patients on warfarin
  • Support and assist occasionally with point of care clinics at Woking Community Diagnostic Hub
  • Patient and carer education, at initiation and throughout the duration of warfarin therapy
  • Formulate guideline led warfarin peri operative plans for ASPH and specialist centres including but not limited to cardiology specialties and bariatric specialties
  • Service development, Make independent professional evidence based decisions requesting and interpreting INR results, including manually adjusting anticoagulation dosages, and be accountable for these decisions
  • Manage and run face to face anticoagulant clinics for complex and clinically unstable patients
  • Provide occasional cover for point of care anticoagulant clinics based at Woking Community Diagnostic hub
  • Formulate warfarin peri operative plans for patients registered with ASPH anticoagulant service and for other specialties including, but not limited to, bariatric services and cardiology services, following national and local guidelines.
  • Manage patients thrombo prophylaxis pre and post operatively as required .Teaching patients to self inject. Commence/discontinue LMWH treatment onown professional judgement based on current guidelines
  • Establish effective and highly complex communication links with all users to ensure safe, efficient and patient centred warfarin management.
  • Provide accessible advice for patients and carers accounting for communication barriers, via telephone, email, correspondence and anticoagulant clinics
  • Provide Warfarin education advice for patients, carers and others healthcare professionals
