Clinical Nurse Practitioner

The successful candidate will also be an active practitioner within the service acting as a role model to other members of staff and eventually providing supporting advice to the team.
You will need to demonstrate creativity, tenacity and a 'can do' approach to service user care.
As well as working within the realm of your own profession you will also be expected to take on a generic role. The role will also encompass research, evidence of service development, audit and supervision of junior staff.
Weekly contracted hours will be 08:00 - 21:00hrs
You will be working in conjunction with the crisis pathway services (Psychiatric Liaison Service, Crisis Assessment Team, and crisis line)
Your role will involve you undertaking face to face comprehensive assessments, including risk assessment, effectively and efficiently.
Formulate and agree crisis safety care plans and to support and advise relatives and care givers.
Whilst your base will be within the Home treatment team there might be need to cover assessments within the Crisis pathway.
The post involves shift work combination of early shifts, late shifts and long days - the applicant is applying is required to have flexibility to work across three different shift patterns., You will have the opportunity to work alongside and within the Mental Health Liaison Team and Crisis Resolution and Home Treatment Team in working towards developing a cohesive crisis service pathway and supporting staffs professional development, experience, skills and knowledge. You will also be supported to develop skills through training provided by the trust to build assessment and formulation skills, risk management of psychotic, depressive, self-harm and suicidal presentation. In house training and development understanding Perinatal mental health, Children and Adolescence and working with individuals with Autism and ADHD.

Our Community
We provide a wide range of community health and mental health inpatient services to children, young people, adults of working age, and older adults in The City of London, Hackney, Newham, Tower Hamlets, Bedfordshire and Luton. We also provide psychological therapy services to the London Borough of Richmond, and provide Forensic services to the whole of North East London. We provide services in urban and rural settings, sometimes in our inpatient units, but mostly in the community close to where people live. Integrated care is key in making the best use of resources and providing effective care.
