Full Job Description

We have an exciting opportunity to join our dynamic prostate cancer CNS team. The team supports patients across Hampshire Hospital Sites including Basingstoke, Winchester and Andover. The band 6/7 clinical nurse specialist post dependant on experience will be required: o To work as an autonomous practitioner in delivering specialist clinical care for patients diagnosed with prostate cancer.

  • To participate in the named prostate cancer nurse of the day rota. To triage GP referrals for suspected prostate cancer pathway.
  • o To work with the prostate cancer CNS team in co-ordinating the diagnostic pathway for individuals referred with suspected prostate cancer, utilising clinical skills to achieve high quality care, in line with national and local standards.o Providing evidence-based care for a caseload of patients alongside the lead prostate cancer consultant. To run nurse led active surveillance/ post prostate cancer treatment clinics. Assessing individuals remotely or face to face utilising various methods of communication.o To act as a key link between the multidisciplinary team and the patient, ensuring communication across professional boundarieso To support with the cancer support worker's clinic activity including workshops and assessment, of those patients enrolled on the prostate cancer personalised stratified follow up pathway., Prostate Cancer Clinical Nurse Specialist To work as an autonomous practitioner in delivering specialist clinical care for patients diagnosedwith prostate cancer.To work with the prostate cancer CNS team in co-ordinating the diagnostic pathway for individualsreferred with suspected prostate cancer, utilising clinical skills to achieve high quality care, in linewith national and local standards.o Providing evidence-based care for a caseload of patients alongside the lead prostate cancerconsultant.
