Full Job Description
An exciting opportunity has arisen to join the busy nursing team as a Colorectal Cancer Clinical Nurse Specialist at Southend Hospital.
You will deliver a patient-centered, clinically effective, proactive and dynamic service from the time of caner diagnosis, throughout patients treatment through to recovery or follow up, or alternatively supporting with palliative care through to end of life.
The friendly team follows a national and local professional agenda, health care policy and the nursing strategy to ensure a shared, forward thinking approach to the delivery of the service.
You will deliver a patient-centered, clinically effective, proactive and dynamic service from the time of cancer diagnosis, throughout patients treatment through to recovery or follow up, or alternatively supporting with palliative care through to end of life.
Promoting / maintaining Colorectal Enhanced Recovery and Surveillance Programmes.
Performing assessments face to face and over the phone as needed of patient symptoms and holistic needs.
Coordinating patient care and advocate for patients and families.
Signposting and counselling support.
Liaising with other MDT's, coordination between in-hospital departments & relaying that information to patients.
