An exciting opportunity has arisen for an experienced Palliative Care CNS to work within the community setting. We are looking for an experienced, autonomous, and innovative nurse to join the integrated specialist palliative care nursing team. This is a Band 7 community-based palliative care CNS post, working alongside, Community Nursing Services, Consultants, Therapists, and wider MDT to ensure that the service and team are providing expert high quality patient focused care to advice and support patients, families and Health Care Professionals. You will be working closely with GP's, Community Teams and Acute Trust staff. As an integral part of community nursing, you will support community teams to feel confident and competent to deliver good palliative and end of life patient centred care and providing expertise for patients with palliative care needs across the borough. Enthusiasm, motivation, compassion, and leadership are qualities and skills considered essential for this specialist nurse role to work in a very challenging environment; you will be expected to have a commitment to and progressive work within the field of palliative care. Additionally, you must be able to demonstrate excellence in clinical practice, acting as a resource for information and education, while working alongside the other specialist services.   • To have a full look at our benefits and what it's like working for us please go here:https://www.clch.nhs.uk/job/pay-and-benefits The Clinical Nurse Specialist (CNS) in Palliative Care will work to empower people with advanced progressive illnesses and those who support them to maximise their individual quality of life and death by enabling the provision of skilled palliative care., The CNS in Palliative Care works as a member of Harrow Community services within Central London Community Healthcare NHS Trust. The team is comprised of a nurse team leader, 6.3 WTE clinical nurse specialists, (one in a development role) one full time specialist palliative care occupational therapist, 3 part time consultants in Palliative Medicine and 1.2 WTE team administrators. Acting as a specialist resource to provide advice, support and information to patients, families and staff, the CNS plays a key role in raising clinical standards in the provision of palliative care services in the community. The CNS will act as a role model and exercise leadership to influence and develop the practice of nurses, medical and social care personnel in palliative and end of life care.
