An exciting opportunity has arisen for a part time Band 6 Diabetes Specialist Nurse to join the Diabetes Service within the Mid Yorkshire Teaching NHS Trust. We have three diabetes centres across the trust at Pinderfields , Pontefract and Dewsbury Hospitals. This post will be based at Pinderfields Hospital but cross site working may be necessary., The Diabetes Nurse Specialist will have responsibility in line with national and local guidelines and protocols to provide assessment of patients needs and advise on appropriate diabetes management. The role will involve empowering patients with the skills and knowledge necessary to attain independence in self-management.

Your role is to effectively manage a caseload of patients with diabetes utilising specialist clinical knowledge/practice.

To work alongside and build relationships with the secondary care diabetes multi disciplinary team colleagues to delivering high quality Diabetes Care

The successful candidate must be able to demonstrate an in-depth knowledge of diabetes nursing. Specialist knowledge will have been gained through previous substantial diabetes clinical experience, and the successful applicant will be able to demonstrate evidence of educating patients and health care staff alike.

Working clinically you will be supported by a team of Senior Diabetes Specialist Nurses and Consultants to develop enhanced diabetes, leadership and management skills.

The role will involve undertaking clinical assessments within hospital and community clinics.

You will be part of a progressive, enthusiastic and dynamic diabetes specialist team and play an instrumental part in the advancement of the service.
