Full Job Description

The post holder will organise and manage a caseload for a defined group of patients / service users, providing specialist nursing support, advice and education and care to that specific group. They will carry continuing responsibility for the assessment of needs, development, implementation and evaluation of programmes of care and the setting of standards within the defined service group.

Raise awareness of Multiple Sclerosis reducing associated stigmas andpromote social inclusion.

Reduce unnecessary admissions and facilitating early hospital discharge.

Work towards practising as a non-medical prescriber within area of Speciality. This will include undertaking relevant training and working within MYHT non-medical prescribing governance framework once completed.

Co-ordinate and manage nurse-led clinics within Speciality to include a relapse clinic and the review of patients treated with disease modifying therapies

Co-ordinate infusion service

Co-ordinate blood monitoring and interpret results

We are looking for a dedicated and highly motivated individual to join the already established Multiple Sclerosis team at Mid Yorkshire Teaching NHS Trust.

You will be joining a long established, supportive and respected team of Neurology Specialist Nurses, who cover a range of neurological conditions. The team is friendly, forward thinking and committed to service development.

You should have recent experience in the field of Neurology or experience nursing patient with long- term health conditions and a commitment to developing your knowledge and skills within the field of Multiple Sclerosis.

Working both as a team member, and independently are necessary requirements. You will be supported by and work collaboratively with Neurologists and Specialist MDT members ensuring effective assessment, planning, implementation and evaluation of care for patients with Multiple Sclerosis.

The role requires excellent communication and influencing skills.
