Full Job Description
We have an exciting opportunity for an additional Band 7 Clinical Nurse Specialist to Join our CNS team. The successful applicant should be an experienced Haemato-Oncology nurse with excellent communication skills and a good understanding of the late effects of treatment along with the potential post transplant complications. The main element of the CNS role is to work as the patient's advocate, to support, educate, inform and co-ordinate the care of patients. In conjunction with the clinical team, undertake risk stratifications for patients having follow-up across the cancer pathway.The post holder will work in the Specialist Haemato-Oncology Team and will be well supported both by their CNS colleagues and wider MDT. CNS's work collaboratively with our Consultant colleagues who are keen to develop the team. There are 13 Haemato-Oncology CNS's at SBH and they all work closely together to provide a wide range of specialist support including nurse led follow up and transplant
coordination.Within Cancer Services at Barts Health there are numerous opportunities for learning and developing skills together with supporting nurses in their professional development.We work closely with the cardio-oncology team, endocrine, renal team, and reproductive medicine.Coordination of the patient pathway is a key element of this role, as is the development of an network of close and effective relationships across the trust and third sector., The post-holder will be based in the Haemato-oncology department at St Bartholomew's Hospital and will enhance the development of the current late effects service and follow up clinics and pathways. The role is focused on patients' experience and the management of chronic illness, with a particular interest in providing expert support to patients who have completed transplant. It is expected that the post holder is a clinical expert in Haemato-oncology care and can act as a resource for staff and an advocate for patients. The post holder will assist in the implementation of Macmillan's Personalised Care Agenda and develop treatment summaries for the patient's pathway. They will work closely with members of the multidisciplinary team to provide advice, and support to patients, carers, and health care professionals.The post holder will focus on the development of the Haemato-oncology Late Effects service and follow up pathways, and will have a particular focus on patients' post
allogenic transplant. They will work effectively with the wider transplant, MD team and with the 13 Haemato-Oncology CNSs at SBH who share an office and work closely together.The post holder will influence the positive experience of patients and carers within the late effects service by working with the multi-disciplinary team across the Trust and acting as the patient's key worker.
