This is an exciting opportunity for a nurse to join the innovative In-Reach team supporting people with learning disabilities who require in-patient care in acute mental health settings. This new team comprises of a Band 7 Clinical Nurse Specialist, band 4 support worker and Consultant Psychiatrist. The team provides expertise in supporting our mainstream colleagues to improve the care given to people with learning disabilities through advising on reasonable adjustments; training; screening possible referrals and formulation. The Band 7 post has responsibility for ensuring high quality care is provided by Oxleas acute and crisis teams for people with learning disabilities. We are looking to recruit a motivated and enthusiastic Band 7 nurse. They also work closely with the MH in LD teams across the three boroughs and Intensive Community Support Team.This is a full time post of 37.5 hours per week and works Monday to Friday working across oall Oxleas mental health units with a base at QMH. The posts also involve carrying out clinical supervision and teaching staff and/or students.,

• Develop formal links with all managers across Oxleas Acute and Crisis and Mental Health inpatient wards.
• Working with inpatient wards to identify reasonable adjustments for admissions who have a learning disability.
• To respond to admissions as they occur and offer support, formulation, and recommendations.
• To coordinate regular meetings with In-Reach Psychiatrist and Band 3-4 nursing associate role to discuss caseload priorities and team development.
• Develop formal links and with all Community Mental Health in Learning Disability Teams to support any admissions of people with learning disability.
• Develop formal links with all local community Mental Health Teams to support any admissions and discharges of people with learning disability.
• To offer specialist training on learning disabilities, reasonable adjustments, and capable environment.
• Line manage a Band 4 specialist support worker who will model supportive interactions with patients admitted.
• Monitor the admission and discharge of patients with a learning disability, to and from, inpatient units; and development of patient information protocols prior to admission. This includes implementing Dynamic Support Register and Care and Treatment Review processes.
• This is an autonomous role, requiring clinical judgement to prioritise/target engagement activity, depending on presenting need and available resources., The Clincal Nurse Specialist provide in-reach support to all in-patient ward within Oxleas. This includes training and advice to relevant stakeholders in both learning disability and mental health services, in anticipation of, or reaction to, hospital admissions for people with a learning disability who meet (or are expected to meet) mental health act criteria.
The post holder will also be responsible for identifying training needs of acute and crisis staff and responding to these in a timely and effective manner. The post holder will ensure people with learning disabilities have access to a range of reasonable adjustments to ensure they receive equal health outcomes compared to their non-disabled peers.

The successful candidates will be an RNLD or RMN with experience working with people who have a learning disability and additional mental health problems.
