Full Job Description

Kings Ophthalmology Service at the South Sites are looking for a Clinical Nurse Specialist with a particular interest in Cornea. This is a fixed term post for 12 months covering maternity leave.
The role involves utilising your knowledge and skills to autonomously manage a cohort of patients as part of a multi professional team, undertake interventional procedures independently and demonstrate the Trust Values.
The service is open 7 days a week and may involve travel and clinic sessions at other Kings Ophthalmology Sites., In collaboration with the care group and speciality team, the post holder is responsible for safe and effective care . They will assess, monitor, implement and coordinate patient management including participating with research, pathway redesign and service improvements projects.
The post holder will play a key role in developing this clinical nurse specialist role and interventions to identify merging issues, reduce patient anxiety, improve efficiency, drive innovation and improve the quality of care through:
+ Providing specialist autonomous nursing care for patients attending an Ophthalmic Speciality Service.
+ Acting as an enhanced practitioner who can lead the specialist nursing function in the delivery of specialist care.
+ A role in facilitating, enabling and supporting service improvement and the development of the CNS remit which is evidence based with best practice for patients so they receive high quality, effective and efficient care.
+ Multi-disciplinary team working to implement a strategic development plan for the service with daily operational and managerial responsibilities.
+ Acting as a resource for staff education and development.
+ Reviewing and managing patient pathways to improve flow and efficiency in the clinic/wards/department to ensure the patient experience is positive while under our care.
The post holder must be an RN and is bound by the NMC Code of Professional Conduct for Nurse, Midwives and Health, 1. To be accountable for their own practice and utilise available opportunities to sustain and improve knowledge and professional competence.
2. Act in accordance with the Nursing & Midwifery Council code of Professional. Conduct
and Scope of Professional Practice and accountable for their actions at all times.
3. Use knowledge and expertise to make clinical judgements using an evidence-based approach and autonomous professional.
4. Request investigations to confirm diagnoses where appropriate.
5. Engage in case discussions and MDT meetings to improve subject knowledge and develop in role.
6. Assess, plan, implement and evaluate specialised programme of care to patients, relatives and carer.
7. Enables patient choice and involvement and initiates appropriate action.
8. To maintain accurate clinical records in accordance with NMC, Trust and medico-legal requirements. To ensure that these standards are met and maintained within the nursing team.
9. To act as a patient advocate when appropriate, ensuring respect for patient confidentiality, privacy and cultural/ethnic background. To ensure that these standards are met and maintained.
10. To be a credible and approachable role model for junior staff and students - providing clinical support and guidance as required.
11. To engage in local and national forums to develop practice and raise the profile of the service and Trust.
12. Provides clinical advice in accordance with current evidence
based practice.
13. Provides clinical advice and support to healthcare professionals with and across the organisational boundaries.
14. Ensures patients and their families are given appropriate oral and written information regarding planned care.
15. To ensure the effective and efficient use of patient resources.
16. To work with other clinical nurse specialists in the service and care group to develop and utilise the pathways and protocols new or already established within the care group.
17. Use previous training to operate equipment relevant to role (egg slit lamp, HVF Analyser, lasers, biometry, icare, optos and Optos scan.
18. Opportunities include: stable virtual clinic reviews, post op glaucoma cataract clinics, interventional laser procedures including SLT/PIs/Capsulotomies, involvement in weekly glaucoma teaching and OPT competency framework.
Management
1. To act as a role model for the nursing and the wider multi-professional team.
2. Manages own caseload and coordinates patient journey.
3. Works in close collaboration with the other health care professionals to ensure the service meets the needs of the patient.
4. To work within relevant trust policy in order to determine and maintain acceptable standards of conduct and performance.
5. To work with the nurse manager to support and when required lead daily operational work and have managerial responsibilities for a cohort of staff.
Quality
1. To facilitate high quality, cost effective care through the use of audit, benchmarking, observations of care and other quality initiatives as determined by the Trust. Ensuring that all feedback where necessary action is taken to improve standards.
2. To attend and participate in multi professional meetings and represent the ophthalmology nursing team as required.
3. To identify any risk to patient or staff safety taking appropriate management action. To support the risk management process through the Datix system.
4. To keep the Nurse Manager / Matron informed of any adverse incidents and complaints. Investigating complaints and adverse incidents as required by the senior nurse, intervening at an early level to resolve informally where possible and implementing any identified outcomes.
5. To participate in the development and implementation of assessment and mentorship/preceptorship processes for new staff to ensure quality standards are met.
