Full Job Description
This is a role within the restructured Palliative and End of Life Care team. It will involve providing organisational and palliative care expertise to improve the quality of palliative and end of life care provided in the out of hospital setting, including acting as an educator to staff, and involving and engaging with senior staff across the Trust to deliver any palliative and end of life care related aspects of the Trust's Business Plan, Clinical Strategy and other priorities as requested. The post holder will be expected to build on existing relationships with hospices, voluntary organisations and other emergency, social and health care providers in relation to developing a collaborative pan London approach to palliative and end of life care. There will be a focus on supporting the development of any education / training / guidance related programmes for staff as the expert advisor on palliative care, alongside consulting on incidents, quality alerts and complaints that require understanding and specialist input. There will be a need to continuously develop current practice and seek opportunities to explore, identify, Improve and monitor high quality palliative and end of life care across the Trust. The post holder will be based at Waterloo Head Quarters, SE1 8SD, but will also be required to travel pan London to education centres and ambulance stations in order to deliver education and training., Providing organisational and nurse specialist knowledge and expertise to provide the delivery of the End of Life Care (EoLC) service to improve the quality of end of life care provided in the out of hospital setting.? Acting as a role model and educator to staff, working in practice alongside operational teams.? Working with the Trust, Clinical Education Department and the Macmillan Paramedic Lead to understand and contribute to the strategic direction of clinical practice and education within the Trust relating to out of hospital end of life care to improve outcomes, staff knowledge and confidence. This will involve working with staff responding to patients inaddition to fostering working relationships with a wide range of external agencies at a local and regional lever to deliver the Service's objectives., Our vision is to deliver outstanding emergency and urgent care whenever and wherever needed for everyone in London, 24/7, 365 days a year.
