Full Job Description
A sensational opportunity has arisen to join the well-established, innovative, and dynamic Respiratory Integrated Team South (RITS) that is part of the Frimley Adult Integrated Respiratory (FAIR) Service.
We are looking for an enthusiastic and knowledgeable B7 Clinical Nurse Specialist/ Specialist Practitioner who is committed to delivering excellent, evidence based respiratory care. You will be part of a senior team, leading and developing nurses, physiotherapists, nursing associates, associate practitioners, physio technicians and administrative support to deliver care for people with respiratory illness in the place of choice. A special interest and knowledge in COPD/Asthma/ILD/Bronchiectasis would be beneficial.
The award-winning team is based at Farnham Community Hospital, but we also see patients in Frimley Hospital, in clinics and in their own homes. The focus is supporting people to manage their condition within the most appropriate setting. No two days are the same!
The job will require working in other designated locations across the ICB as well as the primary base. Flexibility is required across secondary care and community.
At present the role is a predominantly Monday - Friday with the hours being 9-5 (although there might be some requirement to work 8-4 and weekends at times)., The CNS/P will be responsible for the co-ordination and care of patients with respiratory illness to ensure they receive the highest quality care
using advanced assessment skills and knowledge the CNS/P will review patients at home, in clinic or hospital and support the Virtual Ward model to ensure the patients receive the same or improved clinical outcomes and patient experience, compared with an equivalent inpatient stay.
The CNS/P will plan, deliver and evaluate patient care during their period of acute illness, and have advanced understanding of the complexity of issues surrounding the patient and their care.
Using effective counselling/educational skills which may involve breaking bad news on diagnosis or prognosis of a chronic condition. To contribute to palliative care of respiratory patients as they approach end of life.
Assessing and reviewing patients who are on long term oxygen therapy within their usual place of residence or clinics.
Within NEH&F and SH act as an expert resource on respiratory care to patients, health care professionals and the general public.
Provide expert clinical advice, dissemination of information and sharing of experiences with other health care professionals.
