Full Job Description

We are seeking to appoint a Stroke Specialist Support Nurse to come and join our dedicated Nursing and Multi-disciplinary team to support stroke patients on the Ward. More detail about the role : Staples Ward is a bedded adult unit predominantly delivering acute and rehabilitative care to patients who have suffered a Stroke. We also care for a small group of patients that have Neurological conditions and require treatment and rehabilitation. We strive to provide excellent care, with an emphasis on advanced discharge planning by working collaboratively as a Multi-Disciplinary Team. Good interpersonal skills and the ability to work effectively in a team, along with a positive attitude towards change are essential to the role. We plan to develop an inhouse training programme and Stroke Education Passport to provide a structured and standardised approach to education and training for those working within Stroke care. The successful candidate would be expected to support the ongoing training of the team and to provide Clinical teaching and guidance on how to access to a wide range of learning opportunities that will assist the team in obtaining specific Stroke competencies as identified within the Stroke Specific Education Framework. Other responsibilities would involve :

  • Support with essential assessments on the Ward and for patients in ED waiting to come to the ward
  • Reviewing the ongoing needs of Stroke patients whilst on the ward as part of the Multidisciplinary team
  • Secondary Stroke prevention education on BP, smoking, diet and weight
  • Follow up of all patients diagnosed with stroke / TIA for holistic assessment
  • Be involved with referrals to statutory and voluntary organisations such as the Stroke association.
  • Liaise with ongoing services that will be part of patients journey once discharged form Hospital.
  • To ensure the completion of Patients Handheld Stroke passport in conjunction with members of the MDT team.
  • Participate in board rounds, work alongside therapy to plan rehab goals, plan discharge and participate in discharge planning meetings
  • Support the flow of Stroke patients between ED and the ward by facilitating transfers.

Previous working in stroke care would be beneficial.
