Full Job Description
Ø Manages caseload on an ongoing basis including making modifications within prescribed parameters reporting back to the Consultant.
Ø Autonomously performs specialist nurse procedures in line with Trust protocols and NMC guidance.
Ø Provides advice in relation to the care of individual or groups of patients.
Ø Responsible for the effective management of allocated resources.
Ø Responsible for procurement and maintenance of physical assets and supplies for the service.
Ø Demonstrates best practice and relevant knowledge of the NHS and modernisation agenda, seeking to develop the service as part of the professional team.
Ø Contributes to the development of the corporate nursing strategy ensuring local action plans produced, implemented and evaluated.
Ø Represents the Trust at external events, networking with professional colleagues to maximise the opportunities of promoting the Trust and benchmarking practice with peers.
Ø clinical audit
§ 24 our GP notification audit.
§ 25 Appropriateness & timeliness of urgent suspect cancer referrals. Assist in the gathering of information for ongoing audit projects.
Ø Ensuring that all patients have a key worker.
Ø Co-ordination of patient's pathway.
Ø Monitoring of MDT standards in conjunction with MDT co-ordinator.
Ø Implementing evidence-based practice.
Ø request and track relevant investigations post MDT once appropriately trained to do so, Ø Provide physical, psychological and social support for patients/clients diagnosed with An Upper GI condition.
Ø Nurse led Upper GI clinic.
Ø Participate/ organise meetings for Upper GI groups, to enhance the development of Upper GI conditions across Primary and Secondary health care.
Ø Provide educational sessions to multi-disciplinary groups (Medical staff, Physiotherapists, Pharmacists, Occupational therapists and external interest groups).
Ø Project management - improved communication between primary and secondary care
Ø Undertake clinical trials.
