Full Job Description

+ To lead, develop and deliver the clinical pharmacy service to ensure all patients receive outstanding pharmaceutical care
+ To supervise, motivate and support the team to ensure smooth and efficient flow of work.
+ To work with the Pharmacy and wider multidisciplinary teams to deliver quality improvement projects to enhance patient care, efficiency and safety across a number of clinical areas.
+ Provide exceptional customer service, addressing enquiries and concerns of patients, families, and healthcare staff with professionalism and empathy.
+ Work in close coordination with pharmacists and healthcare professionals to ensure proper medication management for paediatric patients
+ Utilise pharmacy software systems to process prescriptions and maintain accurate documentation, + Provide a clinical pharmacy service to allocated clinics to ensure that medicines prescribed are safe, economic and appropriate to individual patient needs.
+ Monitor and review individual patient prescriptions to ensure safe/appropriate prescribing.
+ Provide professional supervision in the outpatient dispensary to protect patient safety in line with the current Pharmacist's Code of Ethics. This includes being able to intervene and advise on the dispensing of outpatient prescriptions and to professionally assess the suitability of each prescription for the patient.
+ Operationally manage the dispensary as required in the dispensing of medicines, utilising the pharmacy computer system for data recording. To ensure the timely supply of medicines to inpatients and outpatients.
+ Contribute to the development of procedures and protocols relevant to the role e.g. clinical pharmacy procedures, relevant dispensary based procedures.
+ Lead, develop and deliver the clinical pharmacy service to ensure all patients receive outstanding pharmaceutical care.
+ Co- ordinate the day-to-day duties, overseeing any additional duties required, identifying any gaps and devising a plan to manage them.
+ Supervise, motivate and support the team to ensure smooth and efficient flow of work.
+ To deputise for the Principal Pharmacist as required
+ Work with the Pharmacy and wider multidisciplinary teams to deliver service improvement projects to enhance patient care, efficiency, quality and safety across a number of clinical areas.
