Full Job Description

The post holder will participate in the provision and development of the clinical pharmacy service to all wards in the Trust in accordance with current Medicines Management Procedures and will work on a rotational basis.
There will also be a responsibility for working in the dispensary.
This role is 37.5 hrs per week plus on call, weekends, Bank holidays and late shifts
In accordance with the current Trust objectives, the post holder will be responsible for ensuring that medicines reconciliations, on the assigned wards, are completed as soon as possible after admission. They will also be responsible for ensuring discharge prescriptions are dealt with quickly and efficiently, responding to medicines information queries and conducting audits
The clinical pharmacist will rotate through a number of specialities every six months. These specialities will include medicine, stroke , trauma and orthopaedics, women and children, surgery, community and out patients.
