Full Job Description
Sidney House & The Laurels Surgery is looking to welcome a qualified and experienced clinical pharmacist into our practice, to play a full and active role within our multi-disciplinary team. The role is offered on a full or part-time basis and the successful candidate will be required to work some of their hours as part of the Extended Access Scheme i.e. between 18:30-20:00 Monday to Friday, or on a Saturday however these hours could be worked remotely. One of the key responsibilities of this role will be to carry out chronic disease reviews, supported by our nursing team.,
- Monitoring patients with complex, long-term conditions such as hypertension, diabetes and COPD, ensuring that medication is optimised, titrated, monitored and complied with.
- Clinically assessing and treating patients, using expert knowledge of medicines, for specific disease areas.
- Providing specialist expertise in the use of medicines and medicine information (e.g. side-effects, dosage).
- Providing pharmaceutical medication review and health education advice to patients, by face-to-face, telephone or notes-based review, as agreed with the practice.
- Supporting the practice dispensary.
- Providing clinical advice and expertise on medication.
- Managing the repeat prescribing reauthorisation process, by reviewing requests for repeat prescriptions & medicines reaching review dates.
- Carrying out structured medicines-optimisation reviews in our care homes.
We are a two-site training practice based in the semi-rural villages of Hatfield Peverel and Boreham, serving a patient population of 11,200.We are a SystmOne Practice with a consistently high QOF achievement and a CQC rating of Good.
