+ Review and manage acute medication requests with a focus on patient safety and efficacy
+ Efficiently process hospital discharge summaries and clinic letters, ensuring seamless patient care transitions
+ Offer expert support and guidance for patients managing self-limiting conditions, empowering self-care
+ Support practices in achieving Quality Outcomes Framework (QOF) targets, enhancing practice performance and patient outcomes
+ Play a pivotal role in enhancing health outcomes for patients with long-term conditions through collaborative care
+ Undertake additional tasks as needed to support the healthcare team and patient care initiatives

Schedules Available: Monday to Friday (No weekends available)
Licence/Certification: GPhC Registration (required)
Job Types: hours per week

+ GPhC registration
+ Independent Prescriber Status (preferred)
+ Minimum of 2 years Post Graduate experience in a GP Practice
+ Proficiency in using SystemOne or EMIS
+ Completion of the CPPE pathway
+ Strong clinical leadership, communication, and interpersonal skills
Desirable:
+ Experience in running chronic condition clinics
+ Proficient in structured medication reviews and acute medicines request reviews
