Full Job Description

The role of the Anticoagulation Pharmacist is to work within the BGPA Community Anticoagulation Service and managing patients on warfarin, as well as clinically screening incoming referrals and practising as a Pharmacist Independent Prescriber and conducting Initiation appointments and prescribing suitable Anticoagulants. Pharmacist is expected to work according to a Standard Operating Procedures (SOP) and PGDs.

This is a part time role, for 4 days a week, to cover clinics in Lewisham and Bromley area and work as a hub pharmacist supporting clinical queries.

Main duties of the job

Support the service according to national standards of practice.

Support multi-disciplinary team of staff within the Community.

Promote standardisation in initiation and maintenance of anticoagulant therapy.

Review patients on warfarin with regard to indication, duration, compliance, target INR, & clinical safety & liaise/refer to appropriate clinicians for medical opinion.

Assess & identify the correct regime for patients outside the therapeutic range & refer.

Provide support for prescribing, dose adjustment & close monitoring following agreed protocols, patient group directions or as an independent prescriber.

Provide specialist advice to individual patients & to other healthcare professionals on the dose of anticoagulants in accordance with agreed protocols

Assess patients condition according to INR & counsel accordingly, taking into account the relevant clinical details.

Identify/follow up patients who fail to attend for INR checks.

Support the management of patients requiring vitamin K.

Counsel patients with unstable or difficult anticoagulant control.

Provide education & training on anticoagulation therapy & monitoring.

Provide anticoagulant services in line with NICE, NPSA & other relevant guidelines.

Assist in thromboprophylaxis management.

Support in management of patients on LMWH

Participate in the domiciliary visiting service for patients on anticoagulant therapy either in person or by supporting ancillary staff by telephone when they are visiting the patient at home
