Full Job Description

Medicines optimisation within the PCN

Medicines Reconciliation

Providing expert advice on clinical medicines

Undertaking structured clinical reviews with patients to manage complex polypharmacy needs

Working as part of a Multi-Disciplinary Team (MDT) in a patient facing role to clinically assess & treat patients

Provide professional advice & support on the safe prescribing, handling & administration of medicines. This includes support regarding development and implementation of medicines management processes & policies

Lead on the medicines aspects relating to the care of patients to optimise medicines, reduce medicine-related risks & improve adherence to taking medicines by carrying out medication assessments & reviews in identified patients

Resolve day-to-day medicines & prescription issues with healthcare providers

Help patients manage their conditions

Lead prescription services & deal with queries

Present to patients any changes to prescribing inline with NICE, best practice or in response to prescribing alerts

Rationalise repeat prescription lists to avoid waste/duplication

Provide reviews to those patients with long term conditions or complex health needs, including drug monitoring & risk v. benefit

F2F & digital appts. to patients

Contribute to the management of the prescribing budget through the promotion of safe, cost-effective rational prescribing

Contribute to minimising the harms associated with the use of medicines

Mottingham, Downham & Chislehurst Primary Care Network (MDC PCN) are looking for an experienced self-motivated, forward thinking qualified pharmacist to join their pharmacy team.

BGPA is a federation of all 43 Bromley practices working collaboratively to enhance the health and wellbeing of Bromley residents, covering over 350,000 patients, 100% of the Bromley population.

BGPAs partner organisation is Bromley Education and Training Hub (BETH). BETH supports education, training and professional development across primary care in Bromley and beyond.
