Mottingham, Downham & Chislehurst Primary Care Network (MDC PCN) are looking for an experienced self-motivated, forward thinking qualified pharmacist to join their pharmacy team. Main duties of the job Medicines optimisation within the PCN Medicines Reconciliation Providing expert advice on clinical medicines Undertaking structured clinical reviews with patients tomanage complex polypharmacy needs Working as part of a Multi-Disciplinary Team (MDT) in apatient facing role to clinically assess & treat patients Provide professional advice & support on the safeprescribing, handling & administration of medicines. This includes supportregarding development and implementation of medicines management processes & policies Lead on the medicines aspects relating to the care ofpatients to optimise medicines, reduce medicine-related risks & improveadherence to taking medicines by carrying out medication assessments & reviews in identified patients Resolve day-to-day medicines & prescription issues withhealthcare providers Help patients manage their conditions Lead prescription services & deal with queries Present to patients any changes to prescribing inline withNICE, best practice or in response to prescribing alerts Rationalise repeat prescription lists to avoid waste / duplication Provide reviews to those patients with long term conditionsor complex health needs, including drug monitoring & risk v. benefit F2F & digital appts. topatients Contribute to the management of the prescribing budgetthrough the promotion of safe, cost-effective rational prescribing Contribute to minimising the harms associated with the useof medicines, To work alongside the management team in assistingwith the running of the Bromley GP Alliance services. Contribute to the management ofthe prescribing budget through the promotion of safe, cost-effective rationalprescribing. Personal / Professional Development Participate in anannual individual performance review, including taking responsibility formaintaining a record of own personal and / or professional development Participate in anytraining programme implemented by the management team Effectively manageown time, workload and resources Assess ownperformance and development and take accountability for own actions, eitherdirectly or under supervision. Communication Demonstrateinterpersonal skills when dealing with all levels of staff across the BGPAusing persuasion, tact and reassurance where necessary. To deal with alltelephone calls related to the services, ensuring callers are communicated within a sensitive and effective manner and problems are dealt with promptly andefficiently. Handleconfidential patient material in a sensitive and discrete fashion in compliancewith Bromley GP Alliance policy and procedure. To be able toinfluence, motivate and involve individuals and teams to reach necessaryperformance target expectations. Liaise with otherdepartments and members of the Bromley GP Alliance and other organisations toensure the smooth running of the office and to assisting the functioning of theteam. The post holder isrequired to follow Bromley GP Alliance policies and procedures which areregularly updated including : Confidentiality / Data Protection / Freedom of Information Post holders must maintain theconfidentiality of information about patients, staff and other health servicebusiness in accordance with the Data Protection Act of 1998. Post holders mustnot, without prior permission, disclose any information regarding patients orstaff. If any member of staff has communicated any such information to anunauthorised person those staff will be liable to dismissal. Moreover, the DataProtection Act 1998 also renders an individual liable for prosecution in theevent of unauthorised disclosure of information. Following the Freedom ofInformation Act (FOI) 2005, post holders must apply the Bromley GP AlliancesFOI procedure if they receive a written request for information. Infection Control All post holders have a personalobligation to act to reduce healthcare-associated infections (HCAIs). They mustattend mandatory training in Infection Control and be compliant with allmeasures required by the Trust to reduce HCAIs. All post holders must complywith the Bromley GP Alliance infection screening and immunisation policies as well as be familiar with the Trusts Infection Control Policies,including those that apply to their duties, such as HandDecontamination Policy, Personal Protective Equipment Policy, safe proceduresfor using aseptic techniques and safe disposal of sharps. Risk Management All post holders have a responsibilityto report risks such as clinical and non-clinical accidents or incidentspromptly. They are expected to be familiar with the Trusts use of riskassessments to predict and control risk, as well as the incident reportingsystem for learning from mistakes and near misses in order to improve services. Post holders must also attend training identified by their manager, or statedby the Trust to be mandatory. Safeguarding children and vulnerableadults Post holders have a generalresponsibility for safeguarding children and vulnerable adults in the course oftheir daily duties and for ensuring that they are aware of the specific dutiesrelating to their role. Smoking Policy It is the Trusts policy to promotehealth. Smoking, therefore, is actively discouraged. It is illegal within Trustbuildings and vehicles. Essential

  • Masters degree in pharmacy or equivalent qualification pre-2002.
  • Registered with the General Pharmaceutical Council
  • Postgraduate qualification in clinical / community pharmacy or equivalent additional experience
  • Evidence of recent continuing learning / professional development
  • Desirable Diploma in clinical / community pharmacy Experience Essential
